Once your appointment is scheduled, please fill out the Medical and Dental History form here. If you prefer, you can fill out the form below. Print and bring it with you to your appointment or email it to our treatment coordinator, Angela, at: patient@parikhortho.com.
during your visit
We always love meeting new patients and their families! You’ll be warmly greeted by our team before Angela, our treatment coordinator, guides you to the patient exam room and introduces you to Dr. Parikh. After we get to know you, we’ll complete a detailed examination in order to answer two main questions: Is there a need for orthodontic treatment? Is this the right time?
Based on this, Dr. Parikh will recommend the appropriate follow-up appointments. If treatment is indicated, the next step is to gather diagnostic records - a set of photographs and a 3D image.

Your records are processed and then used by Dr. Parikh to create an individualized treatment plan. Learn more here.
After a thorough diagnosis and virtual setup, Dr. Parikh will formulate a plan that combines effective appliances and efficient mechanics. With this in hand, you’ll meet for a consultation to discuss the plan and any questions or concerns regarding treatment.
